There's rare as in "hard to find," and then there's rare as in "nonexistent." Some players were wondering which one Khorium, a new mineable metal in the expansion, and the rarest metal in the game, was, after a few of them leveled all the way to 375 mining without seeing one node of it pop up.
Neth first said it was spawning as normal (very, very rarely). Apparently it's a rare spawn from Fel Iron or Adamantite nodes, but players just weren't seeing it come up at all, or seeing it come up very, very rarely-- as in one node every few hours. Finally, earlier yesterday, Tseric acknowledged that the rate was spawning lower than intended, and a hotfix was planned and pushed. When players still complained that they weren't finding it, Tseric said one more fix was on the way, and so Khorium should now be back in the game and spawning as normal.
Now, fixed in three days isn't bad for a bug, and now that it's back in the game, there's really no problem. But didn't anyone in beta hit 375 mining? How did they not catch this one?

I played beta and saw a Khorium node in Hellfire Peninsula. I remember laughing to myself when I saw it... I thought Khorium was such a lame name (and still do).

Actually, Neth clarified that while Khorium can rare-spawn off Fel Iron, it also has its own dedicated nodes.

I saw a Khorium vien within the first 10 minutes of BC release. On my way from the portal to Thrallmart. Didn't think much of it then, but now I haven't seen one since.

I've actually seen a khorium spawn this very morning, in the Great Fissure in Hellfire Peninsula. It looked very shiny. Then I moved on, because my wittle level 61 warrior only has mining 343.

The khorium situation is absolutely horrible. There are dozens of recipes in blacksmithing, jewelcrafting and engineering that require large amounts of the ore, and it's almost totally nonexistent.
I just spent 3 hours day, and 3 hours last night flying over Netherstorm, Nagarand & Shadowmoon and found exactly zero nodes.
Completely unacceptable, imo. If somebody in a virtually empty, high-level zones on an epic flying mount can't find a single khorium spawn in 6 hours then it's broken.
I seriously hope Blizzard isn't sticking with the 'one khorium spawn per zone per day' system they have now. They've made high end craftable items absolutely impossible to aquire.

That's unbelievable that Khorium is so rare. I must have come across 10+ nodes while I was leveling to 62 in Hellfire Peninsula. I was always annoyed that I couldn't mine them as my skill was a measly 315 or so.

Now that's a load of bull. To me, Fel Iron, which you need soooo much of to start, is harder to find than Khorium. Leveling my warrior to 63 in HFP, I saw 2 Khorium vien for every 1 fel iron I found. Now I'm 64, with mining at like 326. A friend of mine maxed out herbilism by 63, another friend got skinning maxed out by 66. Why does Blizz gotta be hatin' on the miners? WTB more nodes per zone.
Also, kinda off topic, but a lot of people pick mining/skinning up to make money. Understandable, even a good idea...in Azeroth. I agree with skinning (I killed it, I skin it), but I think those who use mining also just to make money should stop in Outlands. Blacksmithing is expensive as hell, having to spend almost a stack of ore for one skill at low levels, but when yo have to contend with farmers who just do it for the money, it's even harder.

I've seen 10 Khorium Veins, telling people the location because I cant mine them yet. Maybe I'm lucky? And these are all in Hellfire Peninsula even.

I mined 6 khorium at lunch today. 15 gold per ore = that much closer to my epic flying mount. which means I can fly around faster and farm better and make gold for my guildies to get some mounts too.
